Immunization Requirements
Maryland law requires schools to obtain proof of immunizations in order for children to attend school.
Please refer to the for a complete list of immunizations required for school entry by grade. You can also obtain a copy of the Maryland DHMH Immunization Record (896 form)

Parents are encouraged to call their child’s physician to make an appointment for school-required vaccines. The Baltimore County Department of Health offers vaccines to children without insurance or with other barriers to receiving school-required vaccines.
