If Your Child Requires Medication in School

All prescription and non-prescription medications administered to students in school must be accompanied by a written order signed by a physician/nurse practitioner/dentist and the parent/guardian.

Baltimore County Medication Forms should be completed by the health care provider indicating:
student's name
medication dosage
time frequency
anticipated duration of treatment
side effects and mode of transmission

For any changes in dosage or time of medication administration, a new written order must be submitted to the school nurse.

Medications must be hand delivered to the school nurse or school administrator by a responsible adult and it must be supplied in the original prescription container. Students are not permitted to carry their own medication to school.
